2 – Fill out the Appointment Request Form below and a New Horizons staff member will contact you
3 – If you are unsure which department or program best suits your needs, please call the main information line , 772-468-5600, for assistance
Please have the following information readily available when you contact New Horizons to make an appointment:
– If you have insurance, your insurance company’s name and your policy number.
– If you do not have insurance, a photo ID, proof of residency such as an electric or water bill in your name, and proof of income such as a pay stub, tax return or social security statement.
