Adult Crisis Stabilization Services: At our main campus in Fort Pierce, we have a 30-bed unit that provides adults in crisis a safe place to get treatment and establish balance in their lives again. Children’s Crisis Stabilization Services: Children in crisis have unique needs. Our Frances Langford Children’s Crisis Unit, located at our main campus at 4500 W. Midway Road, gives children and families a safe place for inpatient treatment and the child-centered care required to regain balance in their lives. Mobile Response Team: New Horizons MRT has specially trained mental health professionals who travel throughout the Treasure Coast to assess individuals in crisis, determine level of care needs, and implement therapeutic procedures to promote safety within community settings. Adult Mental Health and Substance Abuse Services
Detox Unit: On our 15-bed unit on Midway Road, adults experiencing acute symptoms from substance abuse can receive medication to prevent and manage withdrawal symptoms and also receive medical and psychosocial treatment to help them become and stay substance-free. Case Management: Adults with a mental illness and perhaps a co-occurring substance abuse disorder need a wide range of services, especially if they lack a natural support system to assist them in their recovery. Through Targeted Case Management, our staff can help them with finding and maintaining supportive houses, establishing linkages with community agencies and services, providing education to families and significant others about mental illness and coping strategies, getting primary medical care at the New Horizons Primary Care Clinic, and overall reducing the need for future psychiatric hospitalization. Intensive Case Management is available for individuals requiring more frequent interventions and a higher level of support to maintain their stability. A team provides services in the home or community setting, with a staff member available 24/7 to assist as needed.
Psychosocial Rehab and Independent Living Skills: We provide psychosocial rehab and independent living skills training in Indian River County, Martin County, and St. Lucie County. We offer recovery-oriented services to assist individuals on their road to well-being. Focus is on skills needed for independent living, such as medication management, pre-employment, health maintenance, social skills and use of community resources.
Children’s Mental Health and Substance Abuse Services
Frances Langford Children’s Crisis Center: Funded initially through the generosity of the Frances Langford Foundation, this 20-bed facility provides services 24 hours a day, 7 days a week for children from ages 6 through 17 who have mental health and/or substance abuse issues and need crisis intervention.
Outpatient Therapy Services Outpatient Therapy Services provide mental health, and substance abuse services for adults and children in our four county areas. Clients may be referred by family, a community agency, the courts, or may be self-referred. Comprehensive social, psychological and psychiatric evaluations are provided. Based upon the evaluation and diagnosis, the appropriate treatment will be provided for adults, children and families.
Case Management: Case managers serve as coordinators for children and their families who are in need of services from many community agencies. A New Horizons team links families to community resources and advocates for the family when needed. They provide services in the home, school and community.
Student Assistance Program (SAP): The Student Assistance Program is a substance abuse prevention conducted in the elementary and middle schools in St. Lucie County to educate youth on the dangers of substance abuse. The educators help students develop greater self- esteem and coping skills individually or in small group sessions. This program is evidence-based. Across the country, this program has proven to be effective. It is funded in part through the Children’s Services Council of St. Lucie and the Department of Children and Families.
Community Action Team (CAT Team): The Community Action Team (CAT Team) is a self-contained, integrated, multi-disciplinary team providing comprehensive, intensive, community-based treatment to families with children and youth at risk of out-of-home placement due to a mental health disorder. The CAT Team consists of: a Team Leader, Mental Health Professionals, Wraparound Certified Case Managers, Therapeutic Mentors, a Psychiatrist, a Registered Nurse, and Support Staff.
The CAT Team provides family-centered, culturally competent services focused around the strengths and needs of each child and his/her family with a goal of supporting and sustaining the child in his/her family system and the community. The CAT Team serves children ages 5 to 21 years of age with a mental health diagnosis or mental health and co-occurring substance abuse diagnosis, at risk for out-of-home placement for whom traditional services have not been adequate. Special Programs
Transitional Group Home (TGH): The Transitional Group Home on our Midway campus is in every way a home for individuals with severe and persistent mental illness who have major skill deficits in activities of daily living and independent living. In this 16-bed supervised setting, they learn to take responsibility for their recovery, practice coping and life skills, gradually gain independence, and eventually transition back to the community. Admission to the home is only through the state hospital system.
Injection Clinic: The Injection Clinic serves clients living with schizophrenia who find that Clozapine is the most effective medication for managing their symptoms. The Clinic follows a drug regime that is tightly monitored according to guidelines established by the FDA.
Psychiatric Services: Psychiatric Medical services are provided to adults, children, and adolescents. These services consist of crisis intervention, diagnostic evaluation, medication management, and other treatment recommendations; such as inpatient or outpatient treatment. Inpatient services provide psychiatric stabilization on the ASU, residential care/treatment, rehabilitation on the Short Term Residential unit, and chemical dependence detoxification. Discharge from an inpatient unit ideally results in seamless transition into the community with referral for follow-up care. Outpatient services are directed towards maintenance of the individual in the community, a focus on recovery and resilience, and prevention of relapse. These services are rendered by physicians, nurse practitioners, and licensed mental health providers.
Primary Care: The Primary Care Clinic provides services to our adult clients to promote educate and prevent basic health issues, while offering diagnostic evaluation for referral. These services are rendered by nurse practitioners and physicians’ assistants with physician oversight & supervision. Health conditions targeted include: weight management, chronic disease management, diabetes, and smoking cessation as well as other primary care needs. This coordinated system combines medical and behavioral services to address the whole person, not just one aspect of his or her condition.
In-Jail Services: New Horizons staff works with individuals in correctional facilities, providing direct services to people who are emotionally disturbed and to those who need help after drug or alcohol-related criminal offenses.
Mental Health Court: Mental Health Court helps individuals with mental illnesses who are charged with a crime in St. Lucie County to navigate the judicial system and receive appropriate services and treatment in the community instead of within the jail system. This innovative program is a collaboration among the judicial system in St. Lucie County, the Public Defender’s office, the State’s Attorney’s office, New Horizons, and area mental health and substance abuse treatment providers.
Short Term Residential (SRT): The Short-Term Residential Treatment (SRT) program has been established to serve the community by offering a multidisciplinary treatment approach to persons who need continued stabilization and an opportunity to re-motivate, re-socialize, and continue on the road to recovery. SRT is designed to empower each individual and support the recovery process to lessen symptoms of mental illness and co-occurring disorders while restoring function to nurture the return to an optimal level of independence and enhance the quality of life.
